COD 117 - A Progressive Mixed Pattern Preceding the CTD Diagnosis

Mixed NSIP/OP patterns typically point towards the likelihood of connective tissue disease and should be mentioned in the report

Bhavin Jankharia

51-yrs old in 2012 presented with a fibrotic NSIP pattern.

The video describes the temporal evolution from 2017 to 2025 with a mixed pattern of fibrotic NSIP, fibrotic OP and DPO/CiOP and a final CTD diagnosis that was established only early this year with evidence of PPF.

Mixed patterns are increasingly being recognized in CTD ILDs and should be mentioned in the report.

This complements the earlier post on Connective Tissue Disease ILD Patterns, where I did not discuss "mixed patterns" in detail and the previous post titled "Mixed Patterns in Connective Tissue Disease ILDs" as well as last month's post titled "Decoding DPO and CiOP"

Case of the Day 116 - Mixed Patterns in Connective Tissue Disease ILDs
Mixed patterns are increasingly being recognized in CTD ILDs and should be mentioned in the report.
Snippet 28: Connective Tissue Disease ILD Patterns
A discussion of the various patterns seen in CTDs. UIP and variants, NSIP, BIP, LIP, PPFE, OP, mixed and rapidly progressive.
Case 116 - Decoding DPO and CiOP
DPO is a pattern…of nodular, mixed or dendriform subpleural ossific areas, a CT marker of CiOP, idiopathic or secondary to other fibrosing ILDs. Clinically, this may be an ILA if asymptomatic or part of an ILD if symptomatic.
