Three recent papers have changed the way we approach ILAs and ILDs. The first was the clinical statement by the ATS on ILAs, which I discussed in the last snippet.

Interstitial lung abnormality (ILA) is becoming an important subject because of a variety of issues, including the increased all cause mortality and progression to ILD. It is important to understand the current status of ILA imaging.

The second is the paper in Radiology titled, "Current State of Fibrotic Interstitial Lung Disease Imaging" by Lydia Chelala and colleagues.

The third is the paper by Chris Ryerson's group on fibrotic hypersensitivity pneumonitis also being a connective tissue disease pattern, which I briefly covered in my Case of the Day on YouTube

There is also an increasing understanding of the role of connective tissue diseases as a significant etiologic factor in ILDs.

This has led me to work on a flow-chart that encompasses this new knowledge as part of a unified approach to ILDs.

I will be using this as a framework to discuss ILDs from now on. I will also be conducting a 16-week REF Classroom on this subject - this will be announced on Sunday or next week, but irrespective of that, I will be working on expanding the concepts in this flowchart throughout this year.
